Product Service Ammunition Incident Report Form
Please use the form below to submit an Incident Report online. You may also use the Download PDF button to the right to download a printable version of the Incident Report Form. Please print and complete that form and return it to Magtech with your ammunition and a copy of your shipping receipt, but only after getting a return authorization number from our technical department prior to shipping the ammunition in question. The return authorization number must appear on the package or Magtech will refuse the shipment.
